Uncemented Total Hip Arthroplasty in Patients with Sequelae of the Infected Hip

Abstract
Purpose : Total hip arthroplasty for sequelae of the infected hip is technically demanding with a high rate of complications. We evaluated the short-term clinical and radiographic results of uncemented total hip replacement for this condition.

Materials and Methods : From May 1993 to April 1999, twenty-one total hip arthroplasties were performed for sequelae of the infected hip. Twelve patients were men and nine were women. The cause of hip infection was pyogenic arthritis in 14 cases and tuberculous arthritis in 7. The average age of patients at operation was 43.1 years(range, 21~60 years). The average follow up period was 3.8 years(range, 2~6.2 years).

Results : The mean Merle d¡¯Aubigne and Postel Score improved to 5.8, 5.6 and 5.5 at the final follow up examnination, from preoperative 3.1, 3.6 and 3.6, in the assessment/criteria of pain, mobility and ability to walk, respectively. The mean leg length discrepancy improved from preoperative 3.4cm(1.6~5.8) to postoperative 0.6cm(0~1.8), with a mean correction of 2.8 cm. Endosteal bone formation around the femoral stem and cup was present in more than 90% of the cases and radiolucency was not present to any discernible degree.

Conclusion : Cementless total hip replacement for sequelae of the infected hip resulted in relief of pain, increased range of motion, improvement in walking ability, and excellent radiographic outcome.
